【】 目的:检测类风湿关节炎(RA)患者的红细胞寿命(RBCS),分析其影响因素。方法:选取40例RA患者作为实验组(RA组),选择同期健康体检者20例为对照组,采用一氧化碳(CO)呼气法检测两组RBCS,对比其差异性。按照RBCS是否缩短将RA患者分为RBCS缩短组和RBCS非缩短组,比较两组患者的临床一般资料和实验室检测结果,分析RA患者RBCS与红细胞参数、炎症的相关性。结果:①RA组RBC、Hb小于对照组(P<0.05),RA组RBCS(77.45±37.25)较对照组(103.40±29.05)明显缩短,差异有统计学意义(P<0.05);②RA患者RBCS缩短组与RBCS非缩短组在性别、年龄、病程方面比较,差异无统计学意义(P>0.05);RBCS缩短组RBC、Hb较RBCS非缩短组要低(P<0.05),但CRP、ESR和IL-6的表达水平均较RBCS非缩短组要高,且统计学均有意义(P<0.05);③RA组RBCS与年龄、病程无线性相关,与RBC、Hb呈线性正相关,与CRP、ESR、IL-6呈线性负相关。结论:RA患者红细胞寿命比健康者缩短约1/4,且与炎性指标水平密切相关,建议临床上检测RA患者红细胞寿命。

【】 Objective:To detect the red blood cell life span(RBCS) in patients with rheumatoid arthritis(RA),and analyze its influencing factors. Methods:A total of 40 patients with RA were selected as the experimental group(the RA group),20 healthy subjects undergoing the routine physical examination at the same time were colected as the normal group.The endogenous breath carbon monoxide(CO) test was used to detect the life span of red blood cells in two gruops and their differences were compared. Patients were divided into the shorten group and non-shorten group based on the RBCS index,general clinical date and laboratory test results were compared. Correlation were analysed between RBCS and erythrocyte parameters,inflammation. Results:①The RBC and Hb in RA group were shorter than that of the normal group(P<0.05),the RBCS of RA patients was(77.45±37.25),which was significantly shorter than that of the normal group(103.40±29.05)(P<0.05).②There were no significant difference in sex,age and the courseofdisease between Shorten group and non-shorten group(P>0.05).The levels of RBC and Hb in Shorten group were lower than those in the non-shorten group(P<0.05),but the levels of CRP,ESR and IL-6 were higher in shorten group than those in the non-shorten group(P<0.05).③The correlation analysis showed that age,the courseofdisease and RBCS had no linear correlation in RA group.The RBCS was positively correlated with RBC and Hb,negatively correlated with CRP,ESR and IL-6. Conclusions:The RBCS in patients with RA is about a quarter shorter than that of healthy subjects,and is closely correlated with levels of inflammation.To test the RBCS of RA patients clinically is suggested.
