目的 探究脑血流动力学(CVHI)监测在颅内动脉瘤患者围术期的应用及护理效果。方法 回顾性分析2020年10月~2021年10月期间本院收治的接受手术治疗的颅内动脉瘤患者60例。根据患者是否接受血流动力学检测将全体研究对象划分为常规监测组(n=33)以及CVHI监测组(n=27);比较两组患者的手术时间、术中出血量以及总住院时间、手术前后S100B蛋白(S100B)、神经元特异性烯醇化酶(NSE)水平、术后并发症发生率、两组研究对象治疗结束后医疗流程、医护讲解、护理服务以及综合体验满意度。 结果 两组手术时间、术中出血量以及总住院时间差异无统计学意义(P>0.05);手术前,两组S100B以及NSE水平差异无统计学意义;手术后,上述指标相比术前出现显著提升,且常规监测组高于CVHI组,差异有统计学意义(P<0.05);CVHI组患者术后发生并发症的发生率显著低于常规监测组(P<0.05);CVHI组患者的医疗流程满意度、护理服务满意度以及综合体验满意度显著高于常规监测组,差异有统计学意义(P<0.05)。结论 围术期血流动力学监测对于改善颅内手术对脑组织的影响具有积极作用、且能够降低术后并发症发生率、提高患者对医疗体验的满意度,具有较高的应用价值。

Objective To explore the application and nursing effect of cerebral hemodynamics (CVHI) monitoring in patients with intracranial aneurysm during perioperative period. Methods A retrospective analysis was made on 60 patients with intracranial aneurysms who received surgical treatment in our hospital from October 2020 to October 2021. All subjects were divided into routine monitoring group (n=33) and CVHI monitoring group (n=27) according to whether patients received hemodynamic tests; The operation time, intraoperative blood loss, total hospital stay, S100B protein (S100B), neuron specific enolase (NSE) levels before and after the operation, the incidence of postoperative complications, and the medical process, medical explanation, nursing service and comprehensive experience satisfaction of the two groups of patients after the treatment were compared. Results There was no significant difference between the two groups in operation time, intraoperative blood loss and total hospital stay (P>0.05); Before operation, there was no significant difference in S100B and NSE levels between the two groups; After operation, the above indexes were significantly improved compared with those before operation, and the routine monitoring group was higher than the CVHI group, the difference was statistically significant (P<0.05); The incidence of postoperative complications in CVHI group was significantly lower than that in routine monitoring group (P<0.05); The satisfaction of patients in CVHI group with medical process, nursing service and comprehensive experience was significantly higher than that in routine monitoring group (P<0.05). Conclusion Perioperative hemodynamic monitoring plays a positive role in improving the impact of intracranial surgery on brain tissue, and can reduce the incidence of postoperative complications and improve patients" satisfaction with medical experience. It has high application value.
